To proceed with the transfer of your Access Consumer Profile (ACP) benefits from your current service provider to Werks Mobile, we kindly request that you fill out the authorization form correctly. This form is necessary to initiate the transfer process smoothly and ensure that your benefits are seamlessly transitioned to our services.

Please ensure that you provide accurate and complete information in the form, including your National Verifiery ID. This ID is essential for validating your identity and ensuring the secure transfer of your ACP benefits. The National Verifiery ID serves as a unique identifier that allows us to authenticate your account and facilitate the transfer without any interruptions.

I understand that by initiating this transfer, I am granting Werks Mobile the authority to access and transfer my NLAD ACP benefits. I hereby consent to the release and transfer of my ACP benefits, including any relevant personal information, to Werks Mobile for the purpose of facilitating the transfer process and ensuring the continuity of my ACP service.

I confirm that I am the rightful owner and beneficiary of the ACP benefits mentioned above and that I have the authority to authorize their transfer. I acknowledgment that, once the transfer is complete, the subscriber will lose my ACP program benefit with my current ACP service provider.
